SOR Registration Forms

Are you registered with the parish?  If not, you can register by picking up a form from the Religious Education Office or by calling the Administration Center at 419-238-3979.

If your child was baptized in a parish other than St. Mary's, please bring a copy of the record with you on the evening of Registration or to the Religious Education Office.  If you don't have a copy of the record, please contact the parish of baptism to request a copy to be sent directly to St. Mary's Religious Education Office, 611 Jennings Road, Van Wert, Ohio  45891. 

The fee for 2011-2012 is $45 per child.  If there is a difficulty in paying the fee, please talk to the CRE, Cheryl Freewalt.

Family Registration Form--One per family

Emergency Medical Form--Pages 1 and 2 per student

When you register your child, please be sure to pick up a copy of the handbook for the year. 

Please register your children yourself rather than sending your child to the Religious Education Office with forms and money.  Thank you.
